New forms: Request for conversion – Group life insurance benefits

A simplified approach for regular groups. A more comprehensive approach for self-administered groups or TPAs (third-party administrators).

News Release

To simplify and optimize the conversion request process, we have replaced the former version of the Request for conversion – Group life insurance form with two new versions:

  1. A version for regular groups (F54-030A), when we have the required information in our systems.
  2. A version for self-administered groups or TPAs (F54-030A-1), when we do not have the required information and must ask the plan administrator to provide it to us.

Here are the improvements made:
If your group is self-administered or if you are a TPA:

  • Members are only required to provide their contact information and sign the form.
  • You provide the following information: reason for conversion, current insurance amounts (up to the maximum amounts eligible for conversion) and dependents on file.

For other groups (which are most of our groups):

  • You no longer need to complete the Request for conversion form, since we have the required information in our systems.
  • Members are only required to provide their contact information and sign the form.
  • Our Administration team provides the following information: reason for conversion, current insurance amounts (up to the maximum amounts eligible for conversion) and dependents on file.

What does not change

Once the Request for conversion form is reviewed, an iA Financial Group agent will contact the member to describe the options available and their cost. Together, they will determine the insurance amounts to be converted.

You can find the two new forms on ia.ca/business/forms under Administration – Forms and Return Envelopes.

Members will find the Request for conversion form for regular groups (F54-030A) on ia.ca/individuals/find-a-form/group-insurance under Administrative Forms. The form for self-administered groups and TPAs is not available at this location to avoid confusion.

Notice for self-administered groups and TPAs
You can provide the right form to members when required.

If a member completes the form F54-030A and sends it to us, we will accept it. However, we will send you the form F54-030A-1 and ask you to complete page 1 and return it to us. This will allow us to process the request.
