Important Notice – Basic Dependents’ Life Insurance

News Release

This communiqué applies to you only if your group insurance contract includes the basic dependents’ life insurance benefit.

If basic dependents’ life insurance (providing coverage for a member’s spouse and/or dependent children) is included as part of your group plan, the following will apply effective August 1, 2014, pursuant to the provisions of your group contract:

  • New plan members, as well as existing plan members requesting a change of coverage, will be required to cover their dependents under the basic dependents’ life insurance benefit. This requirement is applicable regardless of the specified coverage for the health and dental benefits (individual, family, waived coverage, etc.)*.

The Enrolment Request form (F54-018A) and the Change Request form (F54-070A) are being amended to reflect this contract requirement. We will soon make available to you the updated versions of these forms.

Note that most of our forms are available online on our website at inalco.com. From the left-hand side menu, under Forms and documents, click on Group Insurance to find claim and administrative forms as well as other useful documents.

As a reminder, if you use Web@dmin for transactions, you do not need to submit forms for enrolment requests, change requests, or appointment or change of beneficiaries. If you do not use Web@dmin, please send us photocopies of the forms. In either case, be sure to retain the originals because you may be required to provide them upon request.

* At this point, there are still some dependents with waived health/dental benefits who may not have basic dependents’ life insurance coverage, even though this benefit is mandatory under the group contract.
