Update: Suspended Operations at Canada Post

News Release

As of June 14, 2011, Canada Post has suspended operations across the country. Until further notice, we have retained all correspondence normally distributed by regular mail, except for disability benefits, which will continue to be sent by courier service.

Plan members

Direct deposit

You are encouraged to enrol for direct deposit and e-notification of benefit payments. With both services, reimbursements are deposited directly into your bank account and you are immediately notified by email when your claims are processed.V

To enrol, please print the following forms and send them to the fax numbers below:

Claims

To submit a claim for medical expenses and dental care, complete the appropriate request form and send it to one of the following fax numbers:

Please note that direct deposit forms, claims and any other confidential information should only be sent to us by fax or by courier service. To find the nearest office, please refer to our address list.

If you prefer not to enrol in direct deposit, disability benefits will be sent to you by courier service. As for reimbursement of medical and dental care expenses, they will be sent to you once the postal strike is over.

You may contact us by telephone, email and fax. Dedicated and experienced resources are ready to answer your questions. Call us toll-free across Canada. Our phone system will allow you to easily reach our various departments:

We can arrange for claim reimbursements, benefit cards and billing statements to be sent to your office using a courier service once a week for the duration of the strike.

If you need to send us information, you can do so by fax or email to the appropriate department mentioned above. Please note that confidential information (for example claims or direct deposit forms of disability benefit) should only be sent to us by fax or courier service. To find the nearest office, please refer to our address list.

If you don’t already receive your billing statements electronically and would like to make the switch, please call us and we will assist you with the necessary arrangements. Note that premium payments are due on the first of each month. Your monthly payments can be received quickly and easily by choosing one of our electronic methods:

Internet

You can make payments over the Internet through most large financial institutions using the following procedure:

    1. Register for the Internet payment service with your banking institution. 
    2. Go to your institution’s website and choose Industrial Alliance Insurance and Financial Services Inc. from the list of payees. 
    3. When you make your payment, enter the reference number in the space provided. Your personal reference number is composed of the prefix “GR”, followed by 10 digits that make up your policy and your account number.

Preauthorized payment

To use this payment method, you must obtain the enrolment form from your Industrial Alliance group account executive.

Please remember to: 

    1. Fill out the form 
    2. Indicate your policy number 
    3. Send the form with a scan of a void cheque by fax 

This is a great opportunity to encourage your plan members to enrol in direct deposit and e-notification of health and dental claims. With both services, claim reimbursements are deposited directly into their bank account and they are immediately notified by email when their claims are processed. Plan members can also enrol in direct deposit of disability. All they need to do is:

  1. Go to www.inalco.com 
  2. Go to Group Products and Services/Group Insurance/Member Services and To Download Forms 
  3. Select Direct Deposit of Disability Benefits form 
  4. Print the form, fill it out and send it to us with a scan of a void cheque by fax
