Travel insurance: the F54-871A medical questionnaire should no longer be used

The F54-871A medical questionnaire concerning the state of health of plan members traveling outside of their province of residence should no longer be used. From now on, members wishing to confirm the eligibility of an existing medical condition or illness must first contact the Industrial Alliance call centre to verify their coverage. They will then be transferred to CanAssistance, where they will be asked questions about their state of health. In most cases, CanAssistance can confirm eligibility of a medical condition or illness directly by phone, without having a questionnaire filled out by an attending physician. This change will improve the process and provide better service to plan members.

The F54-871A form is now obsolete and we ask you to destroy any that you have in stock.

Please note that members must not call CanAssistance directly because Industrial Alliance must confirm coverage to prevent the unnecessary study of a medical file.
